Precise metabolic care for your patients
We help patients improve type 2 diabetes, prediabetes, and obesity by addressing the root cause: a dysfunctional metabolism.

By combining personalized, data-driven interventions with dedicated clinical care, we’re focused on improving a patient’s overall health while safely reducing reliance on certain medications. Our approach has been validated by peer-reviewed research, most recently in the New England Journal of Medicine Catalyst.
The Cleveland Clinic study
In a study of adults with type 2 diabetes (median diagnosis: 9 years), Cleveland Clinic evaluated Twin Health's approach in a real-world primary care setting.
71%
lowered A1C below 6.5%
without glucose-lowering medications (except metformin)
66%
normalized hypertension
87%
improved inflammation (hsCRP)
78%
improved visceral adiposity
The Twin Health group also experienced an average weight loss of 27 lbs,
compared to 14 lbs in the usual care group. These results further support
our hyper-individualized approach to care.

Frequently Asked Questions
Success depends on many factors, such as health history, current health status, medications, adherence to Twin recommendations, and how the member’s body responds. Our focus is on long-term success and creating sustainable lifestyle changes.
Twin’s type 2 diabetes program addresses elevated blood sugar from the root cause — a dysfunctional metabolism. A one-size-fits-all approach doesn’t work for everyone, because everyone’s metabolism is unique. We begin by learning about a person’s unique metabolic profile, and from that learning, we teach them what to eat and the foods that work best for their body. Additionally, we focus on other wellness areas that are crucial for metabolic health and glucose control, such as activity, sleep, and stress management, all individualized based on the member’s body.
Our Healthy Weight program addresses weight loss from the root cause and recognizes that every metabolism is unique, not a one-size-fits-all approach. We begin by educating the patient on personalized nutrition while highlighting foods that work best for their body. In addition, we focus on other wellness areas crucial for weight loss and maintenance, such as activity levels, sleep quality, and stress management — all individualized based on their body’s needs.
Yes, and we are currently enhancing our offerings in this area. Members will be able to track their workouts, noting their duration and frequency, and schedule them for the week. They’ll also gain insights into how their body responds to each workout.
Type 2 diabetes members receive lab work every 90 days. Healthy Weight members receive labs every 6 months.
While Twin does not encourage alcohol consumption due to its lack of nutritional value, we understand that each member has different preferences and timelines for achieving their goals. In the early stages of the Twin program, members may need to stop drinking alcohol, but as with anything, it all depends on how their body responds.
There will be occasions when members crave a drink or a favorite food, and we’ll guide them on how to enjoy these while staying on track. As the member progresses with Twin, they’ll find that they can incorporate more food and drink options with fewer negative effects.
Twin uses a food coloring system to help members know which foods to eat and which ones to limit or avoid to help them heal their body and achieve their goals. Food colors are based on the nutrient quality of the food and the member’s individual response to that food. Foods that are color coded “green” in the app are the foods that are recommended for the member, based on high nutrient quality of the food and a positive response to that food. Foods that are color coded “orange” or “red” in the app are foods that are lower in nutritional value and/or cause a negative response. Twin helps members find green foods that will help them heal and achieve their goals, while working on moderating orange and red foods for a balanced sustainable approach.
A member’s progress depends on how consistently they adhere to their daily tasks and nutrition plan. The more they follow the program, the faster they’ll see results. On average, it takes a few months for their metabolism to adjust and tolerate these foods without hindering their goals.
Absolutely! The Twin app can assist with this. Members can use the Shopping Guide to filter foods by stores and brands in different categories. This helps them find affordable, locally available “green” foods at places like Aldi’s, Walmart, and other local stores quickly and easily.
Yes, Twin coaches are trained to help members support their fast while eating the right foods. The Twin app can help them choose foods that will make them feel fuller longer and manage their blood sugar to lessen some of the side effects and cravings they may have with extended fasting.

Twin isn’t keto. While some initial “green” foods might be labeled as keto due to their low sugar and carb content, we mainly focus on:

  • Resetting the metabolism
  • Reducing inflammation
  • Steadying blood sugar

As the member’s body heals, foods with sugar and carbs can be reintroduced into their diet. Unlike keto’s one-size-fits-all approach, Twin offers a personalized and sustainable evidence-based approach. We use information from sensors, lab work, and meal logs to help the body heal for life!

As a member’s health metrics improve, we aim to reduce their reliance on medications. This may include the reduction or elimination of medications, including SGLT-2 inhibitors and GLP-1 drugs. The aim is to support the body’s natural healing process, leading to sustained improvements and stability without long-term reliance on medications (where appropriate). However, we recognize that certain conditions, such as atherosclerotic cardiovascular disease (ASCVD), congestive heart failure (CHF), or chronic kidney disease (CKD), may require continued use of certain medications, and we follow evidence-based guidelines to optimize care. We also provide a ketone meter for those on SGLT-2 inhibitors to track ketone levels for 30 days — this is to reduce the risk of euglycemic diabetic ketoacidosis (DKA) when combined with a low-carbohydrate diet. We have had tremendous success in transforming the health and well-being of individuals with our member-centric and comprehensive program.
Twin Health does not replace the role of the primary care provider (PCP). We focus on improving metabolic health and supporting blood sugar management for those with type 2 diabetes, prediabetes, and weight-related concerns. We do not adjust medications outside of those related to diabetes and/or weight loss. Any lab results and medication eliminations will be communicated via fax as shared by the member. We notify PCPs via e-fax regarding any medication changes. We also have a very strong peer-to-peer utilization management program if an external PCP would prefer a more connected communication approach.
