This Notice of Privacy Practices (this “Notice”) tells you about the ways we may use and disclose your protected health information (“medical information”) and your rights and our obligations regarding the use and disclosure of your medical information. This Notice applies to Twin Health Medical Group, P.A. (the “Medical Group”) and other entities related to the Medical Group, including its providers and employees, as well as certain Medical Group vendors and related entities, such as Twin Health, Inc. (the “Manager”) (collectively, the “Practice”).
We may contact you via different methods such as mail, SMS, phone, email, or through the Twin application. Examples of the types of such outreach are listed below.
Please note that email and text communications are inherently insecure. If and when you initiated your account or otherwise established communication with the Practice, you were advised via the website or app Privacy Policy and Terms and Conditions regarding the risks of email and text communications and agreed to accept those risks.
The following categories describe the different ways that we typically use and disclose medical information, the purposes for such uses and disclosures, and the reasons for such uses and disclosures. These categories are intended to be general descriptions only, and not an all-inclusive list of every instance in which we may use or disclose your medical information. To file a complaint, you may either call, email, or send a written letter. The Practice will not retaliate against any individual who files a complaint. You may also file a complaint with the Secretary of the Department of Health and Human Services. the future.
